Bon Secours Hospice uses a team approach to help patients and their families.
Team Members include:
Doctors provide oversight of all medical care.
Registered nurses provide patient care and case management. The care manager is responsible for nursing care for the patient and education and support for patients and families. Case managers work with other hospice team members to teach the patient’s family how to care for their loved one and to provide support.
Medical Social Workers
Social workers help with social service issues. Specific assistance can include sharing information on financial issues, caregiving and final arrangements as well as counseling, educating patients and families on self-help, and advocacy on patient and family rights.
Chaplains with hospice experience and training help patients and families with spiritual matters. Chaplains can coordinate assistance from churches, synagogues and other spiritual sources. They also work directly with patients and families on spiritual matters.
Home Health Aides
Certified nurse assistants specialize in hospice care. They can help patients with bathing, grooming and other tasks associated with daily living. Aides pay special attention to safety and comfort issues.
Social workers are trained in helping individuals and families handle their grief over the loss of a loved one.
