My current position involves a balance of home and hospital visits. In Home Based Primary Care, we are able to serve patients who are unable to travel to a clinic and can help ensure they receive the care and resources they need. In Palliative, we are able to walk with patients through their end of life journey to help ensure it is a comfortable and meaningful experience.
