Welcome to Charter Colony Family Practice - your Patient-Centered Medical Home. It is our sincere desire to give you the best health care possible. We specialize in compassionate family medicine and provide primary care through all stages of life.
Office hours are Monday through Friday, 7:30am - 5pm. In order to provide you with the best possible care, please bring a list of all current medications, any up-to-date medical information and your insurance card. If you need to cancel your appointment, please contact us 24 hours in advance.
For your convenience, refill requests for medications other then controlled drugs may be requested through your pharmacy, and your pharmacy will submit the request to us. Medication refill requests should be made during normal business hours. We require 48 hours to process refill requests. We are not able to provide medication requests after hours.
Please remember that is most situations, appropriate medical care requires a patient examination before a prescription can be filled.
If you have a question, please call us. We strive to answer every call, however if you reach our message service, please leave your name, date of birth, two telephone numbers and your questions so that we can respond promptly. If at any time you are experiencing a life-threatening emergency, call 911.
Referrals and Medical Records
In order to ensure an effective and coordinated approach to your overall health care, it is important that we receive all relevant medical records from you or the referring physician prior to your appointment. Your records are confidential and will not be released without your signed consent.
For emergencies, our physicians are available to you 24 hours a day. You can call our main office number (804) 423-5050, and after hours you will be transferred to our on-call physician service.
Insurance & Payment Information
We take most insurance plans including Medicare. If a co-payment is required, we collect it at the time of your visit. Self-pay patients must pay in full at the time of service. We accept cash, checks, Visa and MasterCard. If you have questions regarding your bill, please contact our business office at 804-521-9300 or 800-732-7072.
