Whether you are newly diagnosed or have had diabetes for years, we can provide the education and support to help you live with diabetes. We provide a comprehensive interactive approach to teach you how to successfully manage your diabetes through education, nutrition, and exercise.
Diabetes educators provide individual and small group sessions on topics such as: what diabetes is, healthy eating, being physically active, testing your blood sugar, taking your medicines, problem solving, reducing risks, and strategies for healthy coping.
Registered dietitians offer individual counseling and group education classes that promote healthful food behaviors and meal planning. They address the challenges of weight control, help you to modify your food choices, and keep you up to date on the latest diabetes and nutrition information.
Physical activity is a key component in diabetes care. Our exercise physiologists will help you develop an activity plan that will help you meet your weight loss goals, address your fitness needs, and improve your health. A complementary one-month membership to the HealThy Self fitness facility is provided upon completion of the program.
Diabetes in Pregnancy Program
This program is for women who have diabetes, or who are diagnosed with gestational diabetes. It provides instruction on diabetes care during your pregnancy and training in blood sugar monitoring. You will learn how to make an appropriate meal plan that helps you manage your blood sugar while maintaining the health of your developing baby.

A physician’s referral is required to participate in our diabetes programs, and once we receive it will contact you to schedule an appointment.
